Each cycle traces a square: a four-count inhale, a four-count hold, a four-count exhale, and a four-count hold, repeated for several minutes. You count silently and can picture the four sides of the box, keeping every phase the same length. It needs no equipment and is often reached for in acute-stress moments, which is how it earned the name tactical breathing (Lee et al., 2023).
Box breathing differs from techniques with unequal phases, such as 4-7-8 breathing or extended-exhale practices, because all four parts share the same count. It is not a fast or hyperventilatory method like cyclic hyperventilation, and its breath holds are gentle rather than maximal. Popularised for military and high-performance use, it is a self-directed calming tool, not a medical treatment.

Hyperventilation-based methods use fast, heavy breathing to deliberately drive carbon dioxide down, the opposite of box breathing's slow pacing and holds, which let carbon dioxide rise slightly. The two feel very different and are not interchangeable.
4-7-8 breathing
Both are counted breathing patterns, but 4-7-8 uses deliberately unequal phases weighted toward a long exhale, while box breathing keeps all four phases equal and adds a hold after the exhale as well as after the inhale.
Diaphragmatic (belly) breathing
Diaphragmatic breathing trains where the breath is drawn, deep into the belly rather than the chest, without prescribing counts or holds. Box breathing instead sets a timed, equal-phase structure regardless of where the breath is felt.
Mindful breath awareness (breath meditation)
Mindful breath awareness observes the breath as it naturally occurs without changing it, whereas box breathing deliberately controls the pace and adds holds. One trains noticing; the other trains active regulation.

Not yet supported: large trials, long-term follow-up, effects in clinical populations, or any claim that box breathing treats a diagnosed condition.
Regular box breathing may leave everyday breathing feeling fuller and less effortful. In a small study of healthy young adults, a month of twice-daily practice improved measured lung function, including how much air the lungs can move in and out (Ahmed et al., 2021). This is early evidence from one small group, so read it as a promising signal rather than a settled result.
Box breathing has some measured backing for helping the body settle, though most trials are small and early.
