What if aging well was less about luck and more about what you know and what you do?

There are very few things I remember word from word from my medical school days,
But one line stayed with me.
My professor of obstetrics and gynecology,
A brilliant,
Respected man ahead of his time,
Said this.
Unless contraindicated,
Hormone replacement therapy is mandatory for all women over 50.
I followed that science,
I prescribed with confidence,
And then in 2002,
Everything changed almost overnight.
A large American study called the Women's Health Initiative sent shockwaves through medicine.
Hormone therapy was abandoned by doctors and patients alike,
Including me.
So when I personally entered perimenopause,
I cautiously asked for a low dose to help with symptoms.
The response I got,
Use it only very briefly,
If at all.
I waited,
I went without it and took it for a short while.
And now at 60,
I'm back on hormone therapy and I couldn't be more grateful.
Yes,
I missed out on years of potential protection.
But the science has finally caught up with what many of us suspected all along.
What went wrong with that original study?
Dr.
Eric Topol,
One of America's leading medical researchers,
Puts it bluntly.
He says the medical community and the public got hormone replacement therapy all wrong for decades.
The study had serious design flaws.
It included women who were many years past menopause,
A very different population to women in early perimenopause and beyond.
The type and the delivery of hormones used in the study also differed significantly from what we prescribe today.
For two decades,
Fear replaced facts.
Millions of women lost out.
What does that estrogen actually do?
Most people think of hormone therapy as a relief for hot flushes and night sweats.
And yes,
It helps enormously with symptoms,
But estrogen is far more than a comfort hormone.
A landmark study in 2024 in Nature Medicine describes it as the conductor of the orchestra,
Coordinating balance across your brain,
Your metabolism,
Your heart,
Your immune system,
And your bones.
Without it,
The whole harmony starts to fall apart.
Estrogen supports memory and protects against cognitive decline.
It maintains your blood vessels.
It preserves your bone density.
It regulates inflammation.
It supports energy and metabolic stability.
This is why menopause is not simply a reproductive transition.
It is a full body recalibration.
And the sudden sharp drop in estrogen at midlife can accelerate biological aging across multiple systems at once.
New research from the UK Biobank,
Which is a database of over half a million people.
Shows that women on hormone therapy have measurably slower biological aging across multiple body systems.
Not just symptom relief,
Slower aging at a cellular level.
Who should consider it?
Be aware this is education,
Not medical advice,
And many of you may choose a natural path.
See a doctor,
Ideally someone with specific expertise in menopause.
It's always the right starting point.
That said,
Hormone therapy is appropriate for many more women than currently use it.
Women in perimenopause or early menopause with troublesome symptoms,
Women concerned about bone or cardiovascular health.
And women who want to understand their options.
It isn't suitable for everyone.
Some medical histories,
Including certain breast cancers,
Are contraindications,
And starting later in life requires very careful assessment.
But if you're in perimenopause or early menopause right now,
This is the time to start the conversation,
Not next year,
Right now.
How do we reframe what's possible?
Menopause does not have to be a period of decline.
With the right information and a good conversation with a trusted clinician,
It can be a transition into a powerful new chapter in your life,
One of strength,
Of clarity and longevity.
The fear narrative is being dismantled by serious science.
My hope is that conversations like this one help make sure that science reaches the women who need it.

Be curious,
And advocate for yourself.
