The Four Foundations Eating Disorder Worksheet uses a table metaphor to show how recovery requires balanced attention to physical, emotional, mental, and spiritual well-being. This resource helps clients assess their recovery stability across all four areas and identify where additional support is needed.


Clients complete the four foundations table by rating each “leg”—physical, emotional, mental, and spiritual—on a scale from 1 (very shaky) to 5 (very strong). Therapists can encourage reflection on strengths and areas needing support, then identify targeted strategies to strengthen weaker legs.
This resource supports individuals recovering from anorexia nervosa, bulimia nervosa, binge eating, or other eating disorders who need a comprehensive approach to healing. It’s especially beneficial for clients experiencing distorted body image or low body weight, as well as those in CBT-based, family-based, or medical treatment programs seeking holistic recovery.
