This Sleep Hygiene Worksheet helps clients identify and change daily habits that contribute to poor sleep. By tracking morning and nighttime routines, caffeine intake, and other lifestyle factors, clients can better understand what’s impacting their sleep quality.


This worksheet is best used over the course of several days to help clients track specific habits that may influence sleep—such as caffeine intake, screen time, and evening routines. Encourage clients to fill it out daily and reflect on emerging patterns. Therapists can review completed worksheets in session to guide psychoeducation, introduce sleep hygiene strategies, and collaboratively problem-solve around barriers to rest.
This worksheet is ideal for clients experiencing insomnia, inconsistent sleep schedules, or general sleep deprivation. It’s especially helpful for clients working on anxiety, depression, or stress-related issues that interfere with restful sleep.
